Overweight patients who have localized area of fat removed must we willing to accept a greater chance of contour regularities, and less ideal redraping skin (skin conforming to the new contour of the underlying tissues). Also, patients with skin dimpling (cellulite) prior to surgery will still have it afterwards.

As in any surgical procedure, the most common complication that may result is a hematoma. This is a collection of blood under the skin. In about 2% of all cosmetic operations this collection of blood must be removed in the immediate postoperative period. Small hematoma are simply removed, and treated several days after surgery in an outpatient procedure. The development and Evacuation of a hematoma does not affect the end result of your surgery, but it may Prolong the healing process for a week or two. Complications such as infection or bleeding can occur, but only in the rarest of cases. Each complication can be treated and may result in a prolonged recovery period.

The Procedure

The surgeon makes an incision about a 1 / 4 “length near the site where he wants to remove the fat and inserts a cannula (a thin tube with an opening near the end) into the incision. Carefully, the cannula is moved back and forth under the skin Administering a medicated solution to the immediate area while a Suction device pulls the unwanted fat through the hole in the cannula, a tube down and out of the body.

This procedure can take from 45 minutes to 2 hours to perform depending on the areas treated.

The Healing Process

Following surgery, a tight fitting of elastic gauze dressing will be placed over the area which will help your skin shrink and conform to the new shape of the underlying tissues. In some cases a corset may be recommended to Insure maximum compression at the site of the surgery, and help reduce any swelling and bruising that may occur. Both corset and bandages are normally removed within a week to 10 days of surgery.

There will be some swelling and bruising following surgery which will subside within a month or two. In some cases where the amount of fat removed is greater than the skins ability to shrink, an Uneven or Rippling effect may be seen on the skin. In general, most patients are up and around a day or two after surgery. You can return to work at this point in time but should restrict all strenuous activities for up to a week or two. After surgery, any pain or discomfort you experience can be controlled with medication. Some patients experience numbness or discomfort which subsides as healing occurs. Scars will be barely notice after surgery and will fade over time.

Anesthesia

A local or general Anesthetic is used. In addition to the Anesthesia, various drugs are given to you before, during and after surgery to make you comfortable.

Post-Op Instructions

Warning: Alcohol and tobacco Prolong swelling and are best avoided 2 weeks following surgery.

You will be discharged possibly with a bra and a tensor bandages wrapped around the bra. You may remove the tensor bandages after 24 hours. The bra should stay on for one week unless the doctor adviser you otherwise. You may sponge bath but keep the bra on your chest and dry.

If antibiotics have been prescribed, you must continue to take them until they are finished.

You may experience some pain and discomfort which is easily controlled with medication. For the first week lifting and avoid any strenuous activities. Bruising and swelling are normal after this surgery and gradually subside.

Excessive bruising and swelling particularly on one side should be reported immediately to The Center.

Seven to ten days after your surgery stitches will be removed and, if required, you will be taught how to massage your breasts in order to keep them soft.

It is not unusual for a patient to go through a period of uncertainty and worry about the appearance of their breasts. It should be understood that it is impossible to judge the final results of the surgery until all swelling has subsided. Immediately after surgery you will experience an increased sense of femininity and confidence. Enjoy this feeling.

Complications That May Result From Breast Surgery

RISK OF COMPLICATIONS: SMALL

The most common complication following Breast Surgery is a “capsular contracture” which can occur in cases where a breast implant is used. In essence, a film of scar tissue forms around the prosthesis causing the breast to feel hard. In most cases, this can be alleviated by applying external pressure. This is done by the surgeon at The Center with or without sedation and the breast will return to its natural, soft state. In rare cases, it may be necessary to open the incision to achieve the same result.

Bleeding and infection may also occur but only in the rarest of cases. Each complication can be treated and may result in a prolonged recovery period. These and other obscure complications will be discussed in detail with you during your personal consultation with the doctor.

The Procedure

There are different approaches which vary according to the type of mastectomy and the condition of the breast skin. When there is enough skin, and if it’s loose and thick enough, reconstruction may be achieved. through a simple implant or prosthesis. An incision is made where the lower portion of the breast would normally be. The surgeon then creates a little pocket beneath the skin and muscle remaining and an implant is placed into the pocket.

When there is not enough skin to accommodate an implant, additional skin from another area is needed. This donor skin is called a “flap” which consists of not only skin, but the muscle and blood vessels that sustain it. Often the broad muscle in the back and its overlying skin is used as donor tissue. The flap is detached and moved underneath the skin ( “tunnelled”) to the breast area where it’s suture to the local skin, forming a pocket for the implant. Sometimes, to “flap” is taken from the abdomen which is “tunnelled” to the breast area and folded on itself. If the Mound is large enough, no implant will be needed, though one may be used if desired.

Another technique involves the use of skin or tissue to expand. This balloon-like device is placed beneath the chest muscle and is gradually filled with a saline solution over several weeks. This Stretches the skin, which then can accommodate an implant or prosthesis.

To reconstructive the nipple and areola, additional surgery may be necessary.

Reconstruction is usually performed in a hospital under general anesthesia, so that you’ll sleep through the entire procedure, which may last 2-7 hours. Afterward, you’ll experience soreness and discomfort which can usually be controlled by medication. You may be up and around in a day or two but you should avoid over activity and refrain from overhead lifting for a few weeks. You should be released from the hospital in 2-5 days.

Every effort is made to make Scars as inconspicuous as possible, but they are an inevitable aspect of any surgery. Scars will fade - not completely, but in most cases, they’ll be very acceptable.

Your new breast may feel Firmer and appear flat or Rounder than your natural breast. It will not have the exact same as your previous breast contour nor will it match your other breast. Though it will not have normal sensation, in time, some feeling should return. This is not an easy procedure, but the potential emotional and psychological benefits are substantial and most patients find it well worth it.

You can usually return to work about 10 days after surgery although your breasts may become sore by day’s end. Scars will remain very visible for about a year and then will fade. Although every effort is made to make Scars as inconspicuous as possible, they will be extensive and permanent. But since they are made around and below the nipple, they should not be notice even in low cut clothing.
